Contractor Forms

The Fringe Benefit Fund remittance report shall be submitted by e-mail no later than the fifteenth (15th) day of the month following the preceding month in which hours were worked. For example, June 2015 hours must be received by July 15, 2015. The remittance report should be emailed to: fringes@local392fringefunds.com.

The Fringe Benefit Fund contributions shall be submitted by the employer and received by Fifth Third Bank electronically via direct deposit by the last business day of the month following the preceding month in which hours were worked. For example: June 2015 contributions must be received by July 31, 2015. (last business day in July 2015)
